Home Medication Reviews

Each year around 140,000 people are admitted to hospital due to medication mismanagement and 69 per cent of these cases are avoidable. ¹

A Home Medication Review is where a pharmacist will have a one-on-one session with you to look at all the medications you are taking and how you are taking them, using the specific knowledge and expertise of the pharmacist. This is done in collaboration with your regular GP and is usually done at your home.

Some of the benefits of a Home Medication Review are:
-Will give your pharmacist and GP a complete understanding of all your current medications and how you are taking them, including any complementary medications you may be taking.
-Will help to improve your understanding of your medications.
-Help improve your medication regime, if necessary.
-Help identify any interactions between your medications.
