What is Sinusitis

Rhinosinusitis (commonly known as sinusitis) occurs when there is a combined inflammation of the nasal mucosa and paranasal sinuses. Put more simply, this inflammation and congestion can cause headaches, facial pain or pressure, blocked nose or ears, nasal discharge, and a loss of taste or smell. But, that’s the simple overview… There’s more to it and it’s quite fascinating.

Let’s take a closer look…

The nasal cavity and sinuses are lined with a mucous membrane which consists of ciliated epithelium. Put simply, these are millions of cells with mobile cilia that lie close together like blades of grass.

Between the cilia are individual mucus-forming goblet cells that produce a watery secretion that surrounds the cilia. The thin secretion is then covered by a viscous layer of mucus, in which dirt, dust and pathogens can easily get trapped.

To rid itself of foreign bodies and to protect itself from infections, our respiratory system has an effective, natural self-cleaning mechanism, which is also known as “mucociliary clearance”.

In the liquid layer of secretion, the cilia (remember the blades of grass?) beat rhythmically in a backward and forward motion, and transport the mucus, dirt, dust particles or pathogens trapped in it, towards the throat. Here the mucus is either swallowed or transported out of the body by sneezing and coughing.

When the mucous membrane becomes inflamed as part of a cold, it produces thicker mucus (in a variety of colours!) and the rhythmic activity of the cilia decreases, stopping the self-cleaning mucociliary clearance. The secretion is no longer transported away properly, leading to a blocked nose or congestion.

From a cold to sinusitis

If the mucous nasal secretion can no longer drain sufficiently, the risk of the narrow connecting passages between the main nasal passages and paranasal sinuses becoming clogged also increases. The result is a classic chain reaction: the accumulation of mucus leads to poorer ventilation of the sinuses, which makes it easier for pathogens to spread, leading to acute inflammation of the paranasal sinuses (sinusitis).
